Try either backing up <http://qsapp.com/wiki/FAQ#How_do_I_back_up_or_reset_Quicksilver.27s_settings.3F> and starting from scratch, or creating a separate user account to test and narrow down the problem of excluding file types. I'll take an uneducated guess and say that maybe some settings are corrupt that keep QS from showing the correct indexing catalogue, but it might be unrelated to the exclude-file problem.
