and starting from scratch, or creating a separate user account to test and 
narrow down the problem of excluding file types.

I'll take an uneducated guess and say that maybe some settings are corrupt 
that keep QS from showing the correct indexing catalogue, but it might be 
unrelated to the exclude-file problem.

Regarding the number of items in your custom Desktop catalogue, try to set 
a depth for the trunk of the hierarchy, say 1/2, and adding from there the 
branches as separate catalogues with depth 2/3, instead of setting 4 from 
the root.
