Dear Marilyn

You do need to hold down all 3 keys together simultaneously. 
I put the little finger of my right hand on the insert key .
Then the pointing finger of my right hand on the right window key .  If your
finger does not reach the right window key you need to use your right thumb.
I then use my left hand to press down the x.

If you do not have a right window key then you will need to use 2 little
fingers on the insert and left window  key with your left pointing finger
reaching out to press the X.

What version of jaws do you have?

Not sure when it was implemented, but I am using a recent version of jaws
