New patch builds on Sijie's patch, but also cleans up how ranges are 
constructed. Now the start and end of the range is derived from the set of 
ledgers passed to the range. NOLIMIT has been removed. An iterator is an 
ordered set of non-overlapping ranges. There can be no empty ranges in the 
iterator. ScanAndCompareGC takes all this into accounts. And covers the entire 
possible ledger id range when doing gc.
