For the (2) [~hustlmsp] mention above, in addition to FlatLedgerManager,
current existing ledger manager HierarchicalLedgerManager and MSLedgerManager
also require to be taken into account. Actually this is the migration of ledger
id generation class, I'll handle it in the backward compatibility JIRA.
