This is looking really good! Looks like it is time to seriously
consider converting my docs :-)

Scanning thru the PDF I noticed a few things:
1) On page 36, 39 (and others) the text overflows the table boundaries.
2) In code I notice that typename, typedef, class and other keywords
   are bolded.  Wouldn't it be better to demphasize keywords and emphasize
   the function names, types and other elements that are the point of
   the documentation?  Looks like this is being controlled by the markup
   in the source...
3) I was trying to run make from the sandbox in /examples and it is looking
   for Makefile.def which isn't there.  More generally, is there a list of 
   things I need somewhere to be able to get my environment set up?
