Interesting. In the HTML output, most of the important elements are emphasized 
by links, so (to me) the bolding of keywords just makes it look it a little 
nicer. On the PDF, however, the bold keywords stand out and the important 
elements don't get links, so they don't stick out at all. It should be 
relatively easy to make that sort of thing configurable, and it might be 
valuable to alter the layout for different media.

I just finished wiping out the need for Makefile.def, and have added a basic 
set of links for getting xsltproc up and running in Unix and Windows (on the 
Wiki). Just grab the appropriate xsltproc and you'll be all set: the makefile 
needs no additional configuration.
