Unfortunately, I don't think it's right yet. On the same sample

I tried running the sample document with and without the workaround I
suggested through the updated QuickBook and this is the diff I get of
the generated boostbook

I found no null characters in the output produced by quickbook. The
version with the workaround apparently gives no errors with xsltproc.
So I guess the bug is now on xsltproc...
Time to settle for the workaround?... ;-)
In my case, I am getting:
..\..\..\..\bin.v2\tools\quickbook\test\xxx\msvc-7.1\debug\
link-static\escape.docbook
../../../../bin.v2/tools/quickbook/test/xxx/msvc-7.1/debug/
link-static/escape.xml:90: parser error : Opening and ending
tag mismatch: code line 90 and unparseable
kbook-escape-prefix--><code>udtA(
<replaceable>some-expression</replaceable> )</
----------------
so, it's choking at the empty end tag: </>
If I apply Joao's workaround, then everything's just fine.
Is "</>" valid XML? I assumed it is. Anyway, it seems that
the XML parser indeed is the one getting confused; not Quickbook.
