Yes, Braille Sense upgrades have always been free, which doesn't mean they will always be, but as they are catching up and trying to establish themselves, I don't imagine they'll charge soon. Yes, the upgrade process is easy. The upgrade process for the Apex is a little more involved, but they're putting a new mechanism in place, and it will handle NLS keys, upgrades, and the like.
For HIMS to upgrade their system to use Windows CE6 instead of 5 would be a very major undertaking. I imagine they will upgrade the current firmware to fix bugs and add functionality, but it will still be based on Windows CE 5 instead of 6.
