Here are my responses to each of your three questions, with the questions themselves for easy reference:

1. On average, how many appointments would you have in your Planner each
week?
I usually keep two or three appointments per week in the planner.

2. When an appointment has past, do you go back and delete it, or do you
retain the appointment as a form of record keeping?

I delete past appointments unless they reoccur, in which case I reschedule them.

3. Do you keep planners from previous years?

No, but I'd like to be able to schedule an appointment in the future. This is particularly useful for doctors' appointments, which are often made six months in advance. In general, I think the planner should not be restricted to a single year, although we should still have the option to create separate files for each year.

I'd like to see an option for scheduling daily, weekly, or monthly appointments. With this option, when the appointment time passes, the record would be automatically moved to the day of the next scheduled appointment. This would be a tremendous time saver for appointments such as weekly meetings, which often happen in work environments.
