A wire does not fail like this UNLESS it is flexed severely, at a point where during manufacture, the machine or bozo who stripped the outer back jacket away was so hungover that he / she cut into the inner jacket and conductor. This cable should never have made it into any machine -- whatsoever. How many others are there out there? This is 120 or 240 volts we are working with here, folks. Fire, shock hazard, you name it.
