Whereas when the market makes the rules, health care goes not to those who
*need* it most, but those who want to *pay* the most.

As for "enforcing rules on an unwilling populace", that's where the
democratic process comes in: if the government starts making rules (and
enforcing them by force) that the public doesn't want, the public will elect
a new government.
