BTW, this is not documented in any way in the grace note section of
the NR, as far as I can see.

Alas, David is right. The grace note synchronization issue #34 has
been filed in the tracker 20 years ago (almost to this exact day), and
we haven't found a satisfactory way to solve it, only to ameliorate
its effects.
