Use the latest snapshot of spandsp from
http://www.soft-switch.org/downloads/snapshots/spandsp/, it is currently
version 20080110. A number of bugs have been fixed since the version
you're using, 0.0.4pre16. Remember to recompile callweaver after
changing spandsp.
Start the tcpdump before the fax starts and stop it after the call is
over. The one you sent is useless, there is no SIP in it at all.
