Why couldn't you just cut the film to fit a 4X5 film holder.  Of course, you would have to do it in the dark but with a good jig I wouldn't think it would that hard.

Just custom made cameras (hence my mailing it to this
list), Cirkuts and Al-Vistas. However, Roundshot does
make a 5" version with a 65mm super angulon (I think).
And old aerials, like you mentioned. I don't know of
any 5" roll film holders for 4x5 cameras.
