I can't find anything wrong with it, except perhaps no space between 
"[256]" and the beginning of this line:

***PALETTES PHASE256_0[256]

I tried changing it to this:

***PALETTES PHASE256_0 [256]

... and it loaded just fine.  But then I tried loading your version, and 
it loaded just fine, too.  And I see PHASE256 in the Metric Settings 
palette drop-down menu.  It seems to work.  I'm using Caret v5.614.  
What version are you using?

Can't reproduce the problem.
