You Can Get turned around down there ( I sure do )     the back nut is the lock nut . it loosens toward port , then turn the front one to port to correct the flow of water , then tighten the back one up again. if you are small one person can do it from starboard birth. if not then one there and one in the port lazeret, probably time to clean it out anyway.

Two wrenches, two people, one on each side of engine.  Hold nut (stuffing box) with one wrench and loosen the locking nut with the other wrench.  Tighten the stuffing box to desired degree and then re-tighten the locking nut while holding the packing nut.  That should do it.
