Do you have a better explanation as to why many of the COIN lessons have been forgotten? Personally I think what it comes down to is a matter of the lack of institutional memory and sheer arrogance. Sheer arrogance in the idea that the worlds most powerful military can conquor anything and do it on the cheap, as exibited by many of the senior political leaders in the run up to the invasion. Lack of institutional memory - only a few generals, like the 101st airbourne CO actually instituted COIN related training to the junior officers and senior NCO's before they are deployed to Iraq.
