Puppies need to go:
* After they eat anything
* After they wake up from a nap
* After they play.

Every.  Single. Time.

This might seem like all the time, and it might just be!  But it will 
get better.  You just have to be consistent and smarter than the dog 
(not always easy!)

Also, another tip for the outside training.   If you ask him to "go 
potty", wait around without eye contact, etc. and he starts to pee, say 
"good potty!" consistently until he's done, so he gets the picture that 
this is what you were asking for.  When he finishes, give him a treat 
and over the top praise, even if he hasn't pooped yet, just to drive the 
point home.    He will start to get it over time if you always use this 
pattern.  I've noticed from my dog that she often needs to run a tiny 
bit before she poops.  Maybe a tiny bit of play after peeing (if you 
think he has to poop still) would be an ok thing.  It's up to you...

Heh...I don't know that I've used the words "pee" and "poop" that many 
times in a paragraph since kindergarten.  Go puppies!
