Well, I did know a guy from West Philly who liked raw onions on his steaks,
even though I still think that's wrong.  If I want raw onions, I get a
hoagie.  It wasn't just my neighborhood, though the closer you get to
touristy areas, the closer you get to wealthier areas, and the further you
get from "Little Italy" (South Philly) the more likely you are to find that
it is necessary to specify that you want onions.  I generally view this as
being burdenned with having to teach someone their job.  A guy in a steak
shop should know how to make a steak....you know?  Anyway, I guess I'm a bit
of a fascist when it comes to cheesesteaks and hoagies.  Damn, I miss
Philly.
