Life expectancy is based on a zillion factors, only one of which is
medical care.  Second, doctors in Japan probably haven't become
accustomed to driving Ferraris and patients (customers) in Japan
haven't become accustomed to the quality of care that money can buy.

As far doctors that want to help people ... Hatton said it best: why
would ANYONE not want the best doctor regardless of their motives for
becoming one?  I guess if I need surgery I don't care if my doctor
recycles.

Further, many doctors (that "want to help people") are like lawyers -
they work for peanuts early in their career and then go for the money
later.  As an example, my former PCP was my doc for 10 years (and made
about 40k/yr).  Then he got married, had kids, and now he's got a new
job pulling down major jing.

UHC will not cure freedom of career choice for smart people - ones who
could save your life with their intelligence they're being paid
handsomely for.  Take away their choice and they'll go to law school
or Wall Street, quality will drop, and just watch what'll happen to US
life expectancy in the operating room!
