whenever I get a pinched nerve it usually goes away with heat and
sleep, though it can take a few days. I have had some episodes that
did not, that required physical therapy.

Apart from a whiplash injury a while back  most of these were caused
by poor ergonomics, sitting too long a a computer holding a phone with
my shoulder. Something to bear in mind. Back off of caffeine and any
other stimulants you might be taking, take some ibuprofen and a lot of
hot showers, use a heating pad and if that doesn't help see a doctor,
for real. Some of the non-muscle possibilities are serious -
torticilis and dvt for example.

Be aware that if you do not have kidney problems you can take quite
high doses of ibuprofen for short periods, as much as 3.2 grams a day,

If you suspect some sort of inflammation, ibuprofen is definitely the
way to go and can help pain that even Tylenol-based narcotics will not
touch.
