exactly, and with a procedure. it's easy, but with older patients, who knows
to say no to a scan, a lab test, an extra visit? no one. some doctors game
the system on the edges, some game it all to hell. some don't game it,
perhaps.

and of course, the doctor checking my hearing is not unusual. the last three
months of life - that's where all the money goes. so when do we cut expenses
there? and what do we cut?
