In many cases it does decrease your costs. I can point you to a number of practices which don't take insurance and have an annual fee that pays for the bulk of the patients costs. They routinely advise patients to also carry catastrophic care coverage and the resulting monthly cost to the patient is less than they would have paid under a traditional insurance plan and they receive a higher quality of care.
