I am still trying to get my head around "The Market". What would drive this
market? I pay for swordfish at Red Lobster based on the availability of the
swordfish. Is there a short supply this week? Oh, so sorry sir, if you would
have got hit by the bus last week you could have had that spleen removed at
the years lowest rate to date but this week there is short supply and high
demand for spleen removal. Hang in there kid, maybe next week!

And if you're riding your bike down the street and get hit by a bus?  Then
what?  You're unconscious and need care immediately.  Hopefully someone is
going to call 911 (in the U.S.) and have an ambulance take you to the
nearest facility that can provide the care you need.  In some severe cases,
that can mean a helicopter dispatched to airlift you to another city that
has the facilities you require.  Sort of hard to shop that around.

I'll also say that I agree with your core point for elective procedures and
general diagnostics, but emergency care is another story.
