Hey, if this were just a bf/gf situation, sure.
But if it's fiance, then they have made a commitment to marry.
That adds a lot of weight to the relationship -
unless they break off the engagement, they WILL get married.
In my opinion, that makes them similar to interns -
they aren't licensed M.D.'s yet, but have to behave like them,
not just play doctor.
Evidently, YMMV...
Hmmm, fiancee's can play doctor, can't they?
