Both been there and done that.... But all leads back to UAT (Useability and
Testing).

Even the best plans can be not what the client expects:-)

Sorry about that, it doesn't help. Just that one of our clients, went and
did a focus group test on the application we built and even though what they
came back with was absolute crap (we'll we thought so anyway). We delivered
what they asked for.

I am not sure what it would cost to get a focus group, but it seems that
larger companies are now going for these focus groups for userability and
ease of use based on the opinions that are received.

The problem I have with this, is that these people are usally there just
because they are paid to be. Does that help in anyway? I doubt it does:-)
