The more I think on this the stranger it becomes. Basically, for it to be getting confused between Application.cfm and application.cfm there would need to be at least one of both somewhere in what you are executing. But, even more bizarre, at some point the .class file for one (lets say Application.cfm) would have to be deleted and cause the other (say application,cfm) to be recompiled. Then, when it tries to hit is dopleganger you would get the error. Are you doing a <CFINCLUDE> anywhere of an Application.cfm further up the tree ? Check the case of the actual reference in there as well.
