Damn fine question Tom.
Sounds like you are not on a Wintel server. Should not be a major issue but
stick to some of these rules:
1. Apart from Application.cfm and OnRequestEnd.cfm - keep your filenames in
lowercase, eg:
index.cfm, dsp_myprofile.cfm, act_doitnowplease.cfm
- it should be noted that CF7 (Blackstone) will introduce a whole host of
new event gateway possibilites but if they stick to Application... And not
say application... Then they should be shot. Move everything to lowercase I
say and at least be consistent.
2. Table names. Well that is definitely not a Wintel (SQL Server 2000)
issue. Find out what db server you are using as it can make a huge
difference.
I hope that helps a bit. DSN issues aside it sounds like the database server
is the problem. Even SQL Server 2000 should at least be running SP3 by now.
And (so far) apart from Application.cfm and OnRequestEnd.cfm stick to
lowercase filenames until MM decide otherwise.

I'm probably on the wrong track but I remember having a ton of trouble
getting my web site up and running on a host and though I can't remember
exactly, this error message looks very familiar.
Turned out that in my code my DSN was in upper case and the server wouldn't
recognise it. Changing the dsn to lowercase fixed it.
The server also wanted lower case table names in my database.
Maybe you could try reefstatedb...
