This will work whether the balancer has slipped or not. In fact, it's a good way to find out if it has since halfway between the two marks will be a true TDC. This was brought up some time ago at TC and if memory serves, most cam mfgs make a screw in plug for #1 to act as a piston stop. I'd take out all the plugs, disconnect the coil wire and turn the engine over by hand.
Bottom line, if you align the balancer with the timing mark (assuming it's anywhere near close anyway) and drop the distributor in, it's either going to fire or be 180 degrees out. If it's 180 out, lift it up, turn the rotor one full turn and drop it back in. The exception being a brand new engine, but even cranking it and finding it 180 isn't going to hurt anything anyway.
