we not even give him the heartworm prevention medicine, because she's had

medicine to prevent the heart worms.

This is mostly I think if he has not been tested. If you give heartworm meds
to a dog that has heartworms then that will make them sick. I don't
understand why a vet would not recommend any heartworm preventive for a dog.
