Exchange your 100nF decoupling capacitors by lower ohmic (ESR) types (e.g.
X7R is probably better than Z5U) or increase the values. Use a surge
protector instead of one C. SMD design? There are protectors in the 0805 and
1206 case available. And simply protect your design from gettings sparks
inside (metal case, input protection - large R in series - C's/surge
protectors to GND and so on ...)
