I have had 3-4 dogs over the past 30 years have demodectic mange.  One was
an adult doberman - mange was due to stress of going to a national
speciality and living in a VW van for a week - used Goodwinil (sp?) ointment
and it healed with no problems.  Other three were boxers.  Two as teething
puppies, one as an adolescent.  I have used white iodine, or lavender oil
(get from homeopathic source) and both have worked equally as well.  The
spot looses all it's hair but the hair will grow back.  Two of the bitches
who had the mange were bred.  None of their puppies got it. The only time I
would worry is if a whole litter of pups contracted mange.  Then I would
consider a genetic immune system problem. Interestingly, I have only had
bitches contract mange, and all were cured easily.  I always increase
antioxident supplements, however.  That's Vit C, A & E.  Also kelp  and B
complex can help boost a depressed immune system whatever the cause.
Teething, adolescence, change in environment, vaccinations can all
temporarily depress the immune system, as can a diet that is not meeting the
puppy's needs.
