Try to look at it this way.  This regulation prevents a facility from being financially penalized just because the resident for whatever outcome was admitted and did not make it to the magic hour of midnight.  I know that between our admission process and the amount of care someone gets if they are only here a few hours we earn every penny of that RUG rate!!!!!!   The first time I clarified this with my intermediary was when a resident (dying) had my undivided/skilled attention for 6 straight hours and I just couldn’t believe we wouldn’t get paid for our services.

I have been asked to pass this billing question on for the billers on the group......

 

"I have a question regarding same day transfers for a Part A resident. The resident is admitted and discharged on the same day. In this case, they did receive services, so we filed a claim to receive payment for our charges. Medicare paid at the RUGS score. So now, we have a credit balance because the day is not billable, and therefore was not booked. Should we have received payment for the RUG, or should we only be reimbursed our charges on same day transfers."
