Just as an FYI, you might want to look at the relevant sections on determining Part A utilization days from Chapter 6, "SNF Inpatient Part A Billing," in the new on-line Medicare Claims Processing Manual. Remember, the SNF-12 Manual is being phased out. Chapter 1 of the SNF-12 has already been DELETED from the CMS web site.
