Speaking of restraints, does anyone have an assessment or know where I can access an assessment to complete prior to placing a resident in a restraint.  I developed my own and the nursing staff does not like it because it is in narrative form.  Any help will be appreciated.  You can email me privately at [EMAIL PROTECTED]   Thanks.

No. If resident can get out freely, this is not a restraint. a resident can have l side rail up for mobility support. Put the siderail up on the wall side, if needed. Using 1/2 rails is OK too, if resident can move freely and doesn't get trapped by it. do an assessment trail with resident and document results.{This is FL.}
