It is according to whether they are impeding the resident’s ability to enter or exit the bed.  Use the instructions given with the restraint section of the manual (section P) and follow these guidelines to determine if they are a restraint for each resident.  This should be an individualized assessment.  Do you do side rail assessments on each resident to support use?

We all use the top 2 siderails for all our residents just for mobility.  Do I need to code them on P4?  Thanks.
