Yes, I replied to another email about this issue a few days ago... When I
find a note like this, I write a progress note explaining why impaction was
not coded on MDS, then I have an individual education session with the nurse
who wrote the note.
It generally only takes one session with me to get the point across. ;-)

.... We had a resident - constipated, not impacted - the nurse documented
impaction inappropriately - would you guys document in the nurses's progress
