If you have documentation that states the resident is impacted, and that
documentation is not correct, you certainly need some further documentation
that explains why H2d is not checked. If you don't have that additional
documentation, the surveys have every right to say you did not complete the
MDS correctly and did not provide adequate care for an impacted resident. 

Who writes that additional documentation is less important that getting it
done. I would suggest you explain WHY the original note was wrong rather
than just a statement that it was incorrect.
