Just curious...where do you write your progress notes...in the nurses
notes...or do you have a separate progress note section specific for MDS
related comments?

Hi, Holly,

Yes, I replied to another email about this issue a few days ago... When
I
find a note like this, I write a progress note explaining why impaction
was
not coded on MDS, then I have an individual education session with the
nurse
who wrote the note.

It generally only takes one session with me to get the point across.
;-)

.... We had a resident - constipated, not impacted - the nurse
documented
impaction inappropriately - would you guys document in the nurses's
progress
note to justify why you did not check impaction on the MDS and if so
what
would you write or would you have the nurse who documented
inappropriately
make a note? Thank you in advance.
