Can you believe my DON asked me what needed to be done when I was out this week and I told her my notes needed to be written in the chart. She said OK but didn't know who would be doing them when I was out. I called today and found out NOBODY is doing them beccause the nurse consultant told her they didn't need to be done. It's nice that I do them, but they aren't necessary. There's enough documentation in the chart without my RAP notes. Yes, I do write notes for Quarterlies , 30 day, 60 day, etc. but they are excellent notes and I have been told by Hospice that they supportwhat they need to get someone covered for Hospice. But what about the significant change, annual, 5 day-- admission- when are RAP notes not necessary for OBRA?
It's nice to know what I do is not necessary!
