Why wouldn’t you count the adjustment in medication without a lab order?  The manual plainly states “include written, telephone, fax, or consultation orders for new or altered treatment.”  An adjustment in Coumadin dosage would be altered treatment with or without lab values.

I don't count the Coumadin as a new order if its only an adjustment in the dosage. However, if there is a lab ordered, I'll count it.
