A change in a medication order, including a dosage change, is counted as a day with an order change. The RAI User's Manual instructions are to include "written, telephone, fax, or consultation orders for new or altered treatment." A change in a coumadin dose qualifies as altered treatment as long as it is not the pharmacist who is writing the orders (RAI User's Manual, p. 3-205).
