A renewal order is like your monthly
orders that are signed by the physician. A change in dosage of medication is
counted as a new order.

Why
wouldn’t you count the adjustment in medication without a lab order?
The manual plainly states “include written, telephone, fax, or
consultation orders for new or altered treatment.” An adjustment in
Coumadin dosage would be altered treatment with or without lab values.
