I absolutely count the coumadin changes as ordered, especially in my setting
where they are newly on coumadin having just been converted from IV Heparin.

I usually count it as it represents that a lab was drawn with abnormal
results and/or resident is exhibiting some form of untoward effect of the
drug necessitating a MD contact, order change and follow up observation all
of which represents a level of skilled care.
