Title: Missing Care Plan Date

A facility did a MDS with RAPS.  Resident went to hospital before Care Plan was completed.  Facility tried putting dashes in the date VB4.  Facility transmitted assessment unsuccessfully.  Would be interested what others do to transmit this kind of assessment.
