Claudia,
You understand my point.  In the RAP Review I state that D/C is planned for approximately day 20 or 21. If they are in the building on day 21, you really do need the care plan, even though it will be primarily focused on D/C planning needs.
 
But, if they leave the facility prior to the careplan being done (before day 21), then on the RAP Summary, I answer "No" to the care planning decision, and then use the D/C date for VB4.

Usually if they leave on the am of their 21st day, I don't do a care plan, since that is the due date for the c/p Thanks
