What I have done with this situation is to add a global rap that explains exactly what happened and I make the VB4 date the same as the VB2.  I also add that the resident will be reassessed when and if returns and that the appropriate plan of care will be implemented at that time.  Have not had any problems with this at all.   Sherri  From: Angie Palac To: [EMAIL PROTECTED] Sent: Monday, January 12, 2004 5:53 AM Subject: Missing Care Plan Date A facility did a MDS with RAPS.  Resident went to hospital before Care Plan was completed.  Facility tried putting dashes in the date VB4.  Facility transmitted assessment unsuccessfully.  Would be interested what others do to transmit this kind of assessment.
