I do understand some. FYI: the Mds cord position that I took over, she gave me zero training. I continually search for ways to make my time fruitful and don't turn me into a fruit! So I state the reason why I will not proceed to care planning is that they will be d/c on their 20th day? I have been doing MDS's for 3 yrs. and is a continual learning process, daily. I was an expert so to speak with OASIS in home care, I was there when it was initated. OASIS seem to the point, easier to complete, even though there were so many forms, there wasn't the heavy burden as w typing for hours "working the raps", completing discus, skin, b/b, s/r's, restraints, going to c/p, UR and QA, and transmitting to the state wkly. Not to mention, taking call q4th weekend. Which can diffenenty take me away from my "putting the puzzle together".  I have always had the feeling if MDS was created after OASIS that it would be such a wonderful system. We shouldn't have to spend hours upon hours typing raps. Some of mine or 5-6 pages typed. I always have had the surveyors state that the raps are excellent and of course depending on the complexity of the resident, my care plans can be ? to 30 pages. Rule a thumb for me, q one will always have a fall, skin care plan no matter what. I never combine my pressure ulcer and skin care plans together, nor do I mix urinary inc w/ bowel. I have however started combining my raps that triggered such as cognitive/communication/vision/mood and or behavior. It keeps me from repeating myself to death.
