To my knowledge, there are no mandatory thinning policies.  There are thinning guidelines that are published by the American Health Information Management Assoc. pertaining to LTC. However, it is prudent to  have this list reviewed by  your Corporations or facility before you start thinning.  I agree with you,  many of the records are becoming unmanageable. That's part of why we developed our policy on "worksheets" as our charts were becomig totally unmanageable with all of the records that we were summarizing in MDS and Nurse's Notes. Mark Abrahamson, R.N.,M.A.

Does anyone have a policy or now of regulations in regards to what stays in the permanent chart or what we can thin from the chart.  The charts can only hold soo much paperwork if you know what I mean and we want to make sure we are in compliance.  Any input would be appreciated.
