I would also become familiar with State guidelines.  I
 
In my state (LA), Conditions for Payment state "retain 3 to 6 months on active chart". 
 
ALso, Federal Regs (Standard Operation Manual) states must retain 15 months of MDS on active chart. 
 
The RAI Manual states the Background (Face Sheet) Information generated on the Admit Assesment (01) must be retained permanently on the active chart.
 
As mentioned by Susan, the AHIMA LTC Guideline has an execellent guide which takes into account the Federal Regulations.  Just be sure to check your state for more stringent or specific.

That's part of why we developed our policy on "worksheets" as our charts were becomig totally unmanageable with all of the records that we were summarizing in MDS and Nurse's Notes. Mark Abrahamson, R.N.,M.A.

Does anyone have a policy or now of regulations in regards to what stays in the permanent chart or what we can thin from the chart.  The charts can only hold soo much paperwork if you know what I mean and we want to make sure we are in compliance.  Any input would be appreciated.
